This policy explains what information Rock Creek Wellness, LLC (“Rock Creek Wellness”, “we”, “us”), collects through this website, rockcreekwellness.com, how we use it, and the choices you have. Rock Creek Wellness is a medical clinic at 5401 College Blvd, Suite 203, Leawood, Kansas 66211.
Your health information as a patient
This policy covers the website. Once you are a patient, the health information in your medical record is protected by federal law (HIPAA) and handled as described in our Notice of Privacy Practices. If this policy and that notice ever differ about your health information, the notice applies.
Please do not send medical details through the website’s forms or by email. Our forms are for contact and scheduling requests. To share health information with your care team, use the secure patient portal or call us at (913) 727-7700.
What we collect
Information you give us. When you use our contact or appointment-request forms, we collect what you enter: your name, phone number, email address, your message, your preferred appointment time and the services you are interested in. Your submission is sent by email to our front-desk staff so we can respond, and we email you a confirmation.
Information collected automatically. Like most websites, ours collects some information automatically when you visit:
- Site analytics. We use Google Analytics to understand how visitors use the site in aggregate: which pages are viewed, how visitors arrive, and the type of device and browser, and approximate location (city level). Google Analytics sets cookies to do this. We do not send the content of our forms to Google Analytics.
- Spam protection. Our forms use Google reCAPTCHA, which collects information about your device and how you interact with the page to tell people from bots. Its use is subject to Google’s Privacy Policy and Terms of Service.
- Security and performance. Our web host and content delivery network keep standard server logs (such as IP address, browser type and the pages requested) to keep the site secure and fast.
- Reviews widget. Our testimonials display public Google reviews through a third-party widget, which loads reviewers’ public profile images from Google.
How we use information
We use this information to:
– respond to your questions and requests, and schedule appointments;
– operate, secure and improve the website;
– understand, in aggregate, which information visitors find useful; and
– comply with legal obligations.
We do not sell personal information, we do not share it with others for their own marketing, and we do not use it for targeted advertising.
Who we share it with
We share information only with:
– Service providers who run the website and our communications for us, under confidentiality obligations. These are our web host (WP Engine), Google (Analytics and reCAPTCHA) and our email provider.
– Authorities, when the law requires it or to protect the rights, safety and security of our patients, staff or others.
– A successor organization, if the practice is sold or reorganized. Patient health information always stays subject to HIPAA.
Links to other services
Our site links to services run by other companies, including our patient portal (Charm Health), supplement dispensaries (Fullscript, Calroy, RnA ReSet), patient financing (CareCredit), and our pages on Facebook, Instagram and Yelp. Those services have their own privacy policies, and this policy does not cover them.
Cookies and your choices
You can block or delete cookies in your browser settings; the site will still work. To opt out of Google Analytics on every website, use Google’s opt-out browser add-on. Our site does not currently respond to “Do Not Track” browser signals.
How long we keep information
We keep website form submissions for no longer than 12 months, unless you become a patient. In that case, the relevant information becomes part of your medical record, which we keep as the law requires. Analytics data is retained by Google under the settings of our account.
Security
We protect the site with encrypted (HTTPS) connections and limit access to form submissions to the staff who need them. No website or email system is perfectly secure, which is one more reason to keep medical details off our forms.
Children
This website and our services are intended for adults. We do not knowingly collect information from anyone under 18 through the website.
Your requests
You can ask us to tell you what website information we hold about you, to correct it, or to delete it, by contacting us as shown below. We will respond within a reasonable time. For requests about your medical record (access, copies, amendments), see our Notice of Privacy Practices.
Changes to this policy
When we change this policy, we will post the new version here and update the date at the top.
