Rock Creek Wellness doesn’t bill insurance, and every patient pays us directly. Below is the federal notice about your right to a Good Faith Estimate, word for word, and then how to ask us for one.
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost
Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
- You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- If you schedule a health care item or service at least 3 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.
- If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.

The notice above is the standard wording from the Centers for Medicare & Medicaid Services (CMS model notice, PDF).
How to ask us for an estimate
Call us at (913) 727-7700, ask at our front desk, or send us a message. Tell us which service you’re considering. There’s no need to include medical details.
When you schedule care with us, we give you a written estimate the same day, without being asked. If you ask us what something will cost, you’ll get a written estimate the same day too. It comes on paper or electronically, whichever you prefer.
Rock Creek Wellness, LLC, 5401 College Blvd #203, Leawood, KS 66211. If you need this page in large print or another format, contact us or call and we’ll provide it.
What your estimate shows
It lists each item or service we expect to provide, with its expected charge. If part of your care will be scheduled separately, the estimate says so, and you’ll get a separate estimate for it.
Most of our prices are on our website. Your estimate is different: it’s written for you, it covers the care you’re scheduling, and it’s the document you would need to dispute a bill.
For ongoing care, such as regular visits or a course of treatment, one estimate can cover up to 12 months. If what we expect to provide changes before your visit, we’ll give you a new estimate.
If another provider or facility, such as an outside lab, will bill you separately for part of your care, you can ask it for its own estimate.
An estimate isn’t a contract. It doesn’t commit you to receiving care from us.
If your bill is higher than your estimate
Keep your estimate, or a photo of it. If you lose it, ask us for another copy: we keep one with your medical record.
You can always call us about a bill, and we’ll go through it with you. If a bill from us is at least $400 more than our estimate, you can also start the federal dispute process. Start it within 120 calendar days (about 4 months) of the date on the original bill. There is a $25 fee. CMS explains how to dispute a bill.
Starting a dispute won’t affect the care we give you.
If you have Medicare, Medicaid or TRICARE
The federal Good Faith Estimate rules generally don’t apply to people enrolled in Medicare, Medicaid, TRICARE or another federal health care program. Those programs have their own protections. If you’re enrolled in one, you can still ask us for a written estimate of our charges before you book.
